Family Carers offers a specialist re-ablement service. This is a 'time limited'​ service, usually six weeks. The service is designed to facilitate early release from hospital or prevent admission to hospital or residential care. The service provides domiciliary support and resettlement services to support disabled people or people with learning disability or mental illness with an assessed need to live independently in their own homes and in their local community, by providing person centred, health and social care re-ablement services. Family Contact Centre – Family Carers offers a range of expert services to children and families, which are safe and cost efficient.
